You can download the two pdf forms below and fill them out before your appointment. Bringing the completed forms to your first appointment will save you and your doctor time.
If you take special care of your teeth and they are in good condition, we recommend that you should come to see us for a checkup at least twice a year. If your teeth are predisposed to problems, we may recommend seeing you more frequently. Dr. Hylton will spend your first appointment doing a thorough examination with a full series of x-rays to determine the health of your teeth, gums, and surrounding tissues then give you a treatment plan to achieve or maintain good oral health. Most often, a hygiene visit will be the second appointment.
Our tip: Come in for a consultation and make the next appointment after each visit.
Proper oral hygiene habits are learned early. As soon as your toddler can manage a toothbrush (24-30 months), supervised brushing instructions can create beneficial habits that last a lifetime. Remember, children under 6 should be supervised at all times as swallowing toothpaste can be harmful. Flowertown Dentistry, LLC recommends a first dentist visit somewhere before the third year. Establishing regular visits at a young age has proven to improve the health and stability of adult teeth.

Please bring your insurance card and identification to your first visit. In addition, we accept all major credit cards, and cash. Payment plans may be set up through CareCredit. Please give us a call to receive information on CareCredit to see if you are eligible. Be sure to ask about all the details during your visit.
